Dr Fehintola Contribution to Knowledge

 Dr Fehintola Akintunde’s contributions to knowledge has been in the field of Gynaecological Endoscopy, Urogynaecology, and General Obstetrics.

Gynaecological Endoscopy

Dr Fehintola Akintunde worked with other obstetricians and Gynaecologists in establishing the feasibility and safety gynaecological endoscopy in our locality despite various challenges and limitations (papers xi, xxiv, and xxx). We elaborated on the roles of laparoscopy in the management of patients with anovulatory infertility from Clomiphene Citrate-resistant Polycystic Ovarian Syndrome (PCOS) (paper xxiv). In this study we established the positive impact of Laparoscopic Ovarian Drilling (LOD) on spontaneous resumption of ovulation, pregnancy, and life birth (paper xxiv) and recommended that this option be considered as a first-line management modality in patients with anovulatory infertility from Clomiphene citrate-resistant PCOS

Urogynaecology

We demonstrated that Pelvic Organ prolapse is a common, but under-reported condition in our settings (papers iv and v) and it has a significant negative impact on the quality of life of the sufferers. Very few surveys exist in the developing world, especially Nigeria regarding the use of mechanical device (i.e., vaginal Pessary) in the conservative management of pelvic organ prolapse (paper xxvi).We described the feasibility of Pessary use in the conservative management of symptomatic pelvic organ prolapse in developing countries (paper xxvi). This method has not been fully utilized in our settings and most gynaecologists easily resort to surgical management thereby presenting limited options to patients. With careful patient selection, morbidities associated with vaginal pessary are reduced with significant patient satisfaction. This method of treatment should be considered before settling down for surgery considering morbidities associated with surgical options (papers v and xxvi).

Another aspect of urogynaecology that we focused on was the care of patients suffering from genital tract fistula (Vesico-vaginal fistula). We studied the burden of vesico-vaginal fistula (VVF) in Ile-Ife (paper xvii), and consider the medico-social consequences of Obstetric fistula in Ilesha, Nigeria (paper xxiii and xxxiii). These studies revealed the debilitating effects of obstetric fistula on women in resource-poor settings. They revealed that this condition is predominant in our area and are strongly associated with difficult deliveries especially prolonged obstructed labour at home, mission houses and in Traditional birth attendants (TBAs)centres (papers xviii, xxii and xix). These studies emphasize the need to equip our health facilities with appropriate resources and health-educate pregnant women and their families on the importance of hospital delivery. Our studies underscore the need for advocacy for family planning, advocacy against early child marriage,and early sexual debut (paper i, ix, xiii, and xx). We also advocate for appropriate adolescent-parental communication about safe sexual behaviours (paper xxix).  These efforts will prevent teenage pregnancy which is a major pathway to the development of obstetric fistula (papers i, ix, x, xiii, xx, and xxix). Patients’ experiences during labour and child bearing in the studied healthcare facility was “disrespectful” (paper xii). This was reported as another reason why many pregnant women patronize TBAs with its negative sequelae like VVF and maternal mortality in Nigeria. There is the need to emphasize patient-centred care and inculcate compassionate care into orthodox healthcare delivery in Nigeria to improve its utilization by pregnant women.

General Obstetrics

In the area of maternal and child health, we carried out a multi-country multicentre randomized clinical trial (WHO ACTION Trial) that confirmed the safety and efficacy of Perinatal corticosteroid administration to prevent neonatal complications in preterm neonates in developing countries (paper xxv). We strongly recommend this drug for use in women with threatened preterm labour to prevent problems of prematurity. Other studies in this area confirmed the efficacy of early catheter removal in preventing bacteriuria after Caesarean Section (CS) (paper xxvii) andestablished the superiority of Chlorhexidine over other antiseptics in preventing surgical site infection in patients during elective CS (paper vi). Ectopic pregnancy, higher order multiple pregnancies and malaria in pregnancy are prevalent conditions in our area, hence our focussed on them (Papers ii, xiv, xxii, xxxi). The studies on the pregnancy and foetal outcome of placental malaria in Ile-Ife (Papers xiv and xxii), and prevalence, predictors, and perceived susceptibility to placental malaria, in Ile-Ife (paper xxi) established the role of Intermittent preventive therapy (IPT) and insecticide-treated nets (ITNs) in the prevention of deleterious pregnancy and foetal outcomes from placental malaria. These emphasised the need for these protective measures during pregnancy.We also studied the patterns and predictors of exclusive breastfeeding (paper iii), Findings from this study showed that the majority of women engaged in mixed feeding leading to an increased risk of childhood diarrhoea which is one of the leading causes of infant mortality in the country. Our studies on fertility-related issues (papers vii, viii, and xv) revealed that most patients have a completely different view of the causes of their infertility and this influences their health-seeking behaviour. Healthcare providers need to keep this in mind while dealing with patients with infertility in Nigeria. We also studied issues surrounding adolescent health which included the knowledge, attitude and practice of menstrual hygiene among adolescents in Nigeria (Paper xvi). This study showed very poor knowledge of sexual and reproductive health issues among adolescents in Nigeria and the need for adequate information on this subject matter in our secondary schools 

Pain relief during invasive procedures is the right of every woman. We studied ways to reduce pain during Manual Vacuum Aspiration (MVA) for incomplete miscarriage (paper xxviii) and Hysterosalpingography (xxxii).
